Step 1:  Check you are within the practice catchment area. 

Step 2: Once you have established that you are within the practice catchment area, you can click the link below to get started with online registration.

Alternatively,  you can download the following documents from the bottom of the page or visit the practice to collect the following:

  • Registration form (GMS1)
  • New patient questionnaire
  • An opt-out form for Summary Care Record (if you require it). 

Please note: You will need to bring you passport or other photographic ID and proof of address to register with the practice.

Step 3: Complete the required forms. The registration form will ask you for personal details such as your name, address, date of birth, telephone number, previous address and doctor. This information helps us to quickly trace your medical records. The new patient questionnaire will ask you for some basic information about lifestyle and ethnicity, so we can provide the best service possible from the start.

Step 4: Attend your new patient health check.

It is important that you attend your appointment with the nurse for a new patient check. They will take a medical history from and enter any significant information such as allergies and medications onto your computer record.

Step 5: You are placed on the practice list. Once you have completed and signed the form and attended your new patient check, you will be registered with the practice.
